1. SMS Program: We send appointment confirmations, reminders, rescheduling notices, and other non-marketing appointment-related messages. Message frequency varies, typically 1-2 messages per appointment. Message and data rates may apply. Reply STOP to opt out or HELP for help. To opt in, patients may voluntarily check the SMS consent box on our in-person paper consent form, or explicitly agree to receive SMS messages during a phone appointment request after hearing the SMS consent disclosure. SMS consent is optional and is not required to schedule an appointment or receive treatment.

2. No Sharing of Mobile Data: No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. We do not sell or share your phone number or SMS consent.
